How to obtain general membership

People with any occupation, and of any age or gender are welcome. Anyone can join the Society. All you need to do is to apply for membership and pay the membership fee. If you wish to become a member, please download the application form below and fill in your name, date of birth, gender, occupation (if you are a medical doctor, please state your specialty), postal address, telephone number, fax number and other relevant information; attach proof of payment of the annual membership fee (such as a copy of the payment receipt), and email it to the Secretariat of the Society (details below). You will be granted membership upon receipt of the membership fee by the Secretariat of the Society.

• General membership: Application form

The annual membership fee is 5,000 yen. Date of receipt of the fee will be taken as the date of joining the Society. As soon as the payment of the annual membership fee is confirmed, a "Notification of Completion of Registration" will be sent to you along with your Society membership number. Before joining the Society, you are requested to read the Bylaws that are posted on this website.

How to obtain supporting membership

The annual membership fee for supporting members is 30,000 yen. If you need an invoice, please contact the Secretariat of the Society (details below).
Please fill in the necessary information in the membership application form and email it to us with proof of payment of the annual membership fee (copy of payment receipt) attached. You will be granted membership upon receipt of the annual membership fee by the Secretariat of the Society.
